Macrolide therapy in asthma: limited treatment, long-term improvement.
نویسنده
چکیده
As part of their interesting review of long-term macrolide therapy in chronic inflammatory airway diseases, CROSBIE and WOODHEAD [1] cite eight randomised double-blind placebocontrolled trials of macrolide therapy in asthma. Most were small, and all were short-term (none .12 weeks). Some positive effects were noted; mechanisms proffered included both anti-inflammatory and antimicrobial (including against Chlamydia pneumoniae).

Immunomodulatory effects of macrolide antibiotics - part 2: advantages and disadvantages of long-term, low-dose macrolide therapy.
The available evidence for long-term, low-dose treatment with 14- and 15-membered ring macrolides in non-cystic fibrosis (CF) bronchiectasis, COPD, chronic sinusitis, and asthma is reviewed with special attention to possible adverse effects and the emergence of resistance during long-term macrolide treatment. Macrolide maintenance therapy has been proven to be of benefit in diffuse panbronchiol...
متن کاملMacrolides for the long-term management of asthma--a meta-analysis of randomized clinical trials.
BACKGROUND Macrolide antibiotics, which have anti-inflammatory and immune modulatory effects, have been studied as adjuncts for the management of asthma. However, results have been contradictory and trials underpowered. We therefore sought to conduct a meta-analysis of randomized controlled trials (RCT). METHODS All RCT of prolonged macrolides (3+ weeks) for asthma treatment, published up to ...

متن کاملInfluence of macrolide maintenance therapy and bacterial colonisation on exacerbation frequency and progression of COPD (COLUMBUS): Study protocol for a randomised controlled trial
BACKGROUND Chronic obstructive pulmonary disease (COPD) is characterised by progressive development of airflow limitation that is poorly reversible. Because of a poor understanding of COPD pathogenesis, treatment is mostly symptomatic and new therapeutic strategies are limited. There is a direct relationship between the severity of the disease and the intensity of the inflammatory response. Bes...
